Hi,
I need send off my test pass certificate off for the photcard license now, but i went post office and i was told that i just need to send my provisional license both card and counterpart, pass certificate and fee. But reading the certificate it says i need a D750 form?
Just trying to get it send off asap and need to know what i need to send.

it tells you what to need in the notes you get witht he form. I suggest you read them and work out what you need to send. when I changed address I just sent off one form and my old license. I think it was the D750 form but can't be sure

provisional, counterpart, and a cheque for the paying to make it pink.
Dont need to send the certificate off I dont think like.

My gf did this yesterday. If it's one of the new pass certificates (blue and A4 sized) then you just fill in the back and send that with your card and paper licence and cheque.
When I passed in 2000, the pass certificate was different (looked like an MOT certificate), and you had to send in a D750 with that and your licence.
