Came back to my service, re-filled the coolant, started it up to let it warm up. After a few minutes another tech got me to check the lights on the car he was working on then when I got back to my ramp the car wasn't running Asked if anyone had turned it off and no-one had, ignition lights were on then there was a sudden feeling of dread as I couldn't remember filling the oil up Checked the dipstick, dry Then I spotted the sump plug still on the ramp, lifted the car up, put the sump plug in, topped up with oil, went to start and it ran like a dream Still to this day I can't believe I got away with it

Forgot to do bolts up on cars, Usually not important.
I ripped the wiring out of a crank sensor on something, Can't even remember what car it was, Ended up needing a new loom as couldn't get the plug separate, wasn't cheap.
Boss has had more fuckups than me, Drove for about 40 miles with the front wheels lose on his own car, Drove a van into the back of another car while moving it around.
Most expensive was years ago I think, left a escort cosworth outside and it was nicked by cutting the fence down.

Once we specified a heating pump in a plant room and told them to wall mount it. Unbeknown to me it was absolutely massive so they had to make a box to put it on and drop the pipe from the ceiling down to the floor, hook the pump up then rise the pipe back up again on the other side Have got a picture of it at work.

I've completley decimated a surface table whilst still half asleep on 6-2.
Filled up a press which was fresh from a refurbish with the wrong oil, i didnt tell anyone. Just kept topping it up with the right stuff and hoped for the best (you cant drain it) it was ok
Smashed a few sets of tooling by sending the press over and hoping for the best when i should of really had the dies out and checked.
Run about 40k bullets off in one shift and then realised they had a tiny mark on the canellure and were scrappers
