When watching apprentice lastnight and also noticed this on other tv shows.

Do people use their phone like the tv people do? or is this a requirement for filming

i.e: loudspeaker on and holding phone in front of you, or by the side of your head, but never nearer than 6 inches to your face.

i hold mine to my ear, like people did in the old days.

I use loud speaker all the time but i just put my phoneo n the desk so i can get on with doing what ever it was i was doing.
never hold it if its on loud speaker

I use speaker all the time, because the speaker in the phone is fucked, I still hold it to my head though and have it on really low
