Introduction -
On the 5th October 2009, a Phoenix airways Airbus A320 carrying 95 passengers and crew crashed approximately three minutes after take-off from Coventry airport on a scheduled flight to Pathos, Cyprus. After take-off the aircraft issued a May Day alert and the captain indicated possible handling problems before losing altitude, climbing and then diving for a final time. Weather conditions at the time of take-off of flight 103 were minimal with wind at a speed of 5knots from 090°, visibility over 10km and only broken cloud cover at 3,000 feet [Met Office 2009]. Phoenix airways had six Airbus A320 aircraft and had been operating under commission regulation (EC) NO 859/2008 OPS 1 subpart C, since May 1991 and were therefore licensed to perform commercial air operations. The aircraft in question, G-ALAB, had undergone a replacement of the autopilot pitch servo on the day of the incident. This was due to reports that the pitch was uncontrollable with the autopilot engaged. Initial investigations suggested that this may have been the cause of the crash. In order for the pitch servo to be replaced, the aircraft autopilot approach system was downgraded to category one before being upgraded to category two once again. Once this procedure was complete, the aircraft underwent ground testing and three standard approaches to Coventry airport. The aircraft commander reported no apparent fault. G-ALAB was maintained by Phoenix airways’ own engineers under AlMA maintenance control and all repairs and modifications embodied were carried out by the appropriately qualified engineers.
