what form do i need to take off the private plate on my car and put the original back on?

Dont think you need a form, do you own the plate? Or just bought a car with one on?
From what i remember, you just take the log book off and say you want to take the plate off/put it on retention.

Ah yeah, unsure which form it is, but go to DVLA office, you need logbook, MOT if it has one, and the number on your tax disc, they will give you a form to fill out there and then.
You put car details on, vin number, tax disc number etc, and they sort the rest, takes about a week for them to send you your new MOT and tax disc with the old reg on again, then another day or so for the logbook.

They also send you a rentention form what you would use to put it on another car again.

Edit: Plate assigned should be the one it was given when orignally registered unless it has been used (which would be highly unlikely but i read that on a dvla leaflet/webpage when i first got my plate years ago.
