I've got mine above but it's a very low fire installed as low in the wall as regs allowed.

Bottom, TV is higher than the sofa and your head doesn't go that far above the top of sofa.

I find TVs that are higher up much more comfortable as I don't sit upright on the couch thus the natural angle on my back means that looking 'up' is actually looking straight ahead.

Had mine on the wall for a year or so, then took it off to redecorate and put it on the stand for a bit. Was so much better I ended up keeping it like that.

Even when slouched the natural angle is looking straight ahead at head height, not up.

Agreed, but most living rooms are terribly laid out in terms of an AV setup with stupid window, doors, radiators and fireplaces in the way of the sensible television spots
I'd block a window off though before putting a telly above the fireplace, silly idea - yet soooo many people do it.

Why is it silly? Focal point of the room! If your fireplace is high then obviously you'll get a sore neck.
Mine could have gone lower but sitting on sofa, that's where it felt most natural to view.

Agreed when slouched you look straight ahead but the effect of slouching is your back is not up-right and thus looking straight ahead actually means your looking above the horizontal (but not 'up').

I fucked my fire off completely and put the tv in it's place. Got a top heating system anyway and fire hadn't been on in years.
