I've been into this for a while now and its fooking AWESOME!
Basically you take a large R/C plane and you strap on a camera, add long range radio gear for control & video and fly it via a screen or goggles.
My record so far is 4km but I had a crash on my way back due to a radio glitch (oops) I've now build a new FPV platform which has been a pretty substantial investment
This is my current flying wing, Its 1.4 meters across, I'm using 435MHz for control (effective range of up to 30km!) and 2.4GHz for video.
It looks strange but there are good reasons to use a flying wing.. They weigh less that conventional planes and offer more lift per gram, in adition they are also VERY VERY strong so survive the odd crash.
Its also fitted with an auto pilot system and GPS that will (hopefull) return it to launch if there is a problem.
Heres a couple of videos of my last build, excuse the shit quality and the mic was on the ground with the DVR.

Ali is the nuts he gets loads of free development stuff to test because he does cool vids ...
I've just invested nearly £300 in an autopilot system and I need to spend another £100+ on the radio link.
I think my flying wing owes me nearly £1000.. but dont tell the missus

That has made me want an RC plane with a cam setup. How much would a setup like that set me back?

You can build a basic setup for about £2-300..
There is a ready to fly setup called the Spyhawk but its SHIT.
