Ideas on how to create an electronic questionnaire?

Just needs to have a set of questions with a set of answers and then stats which I know how to pull as I use access as a database with stats but never built a questionnaire

Can't you use a site like Survey Monkey?

You can set up an online questionnairre and it gives you all the results etc.
