I'm stuck on a mission where the objective is to 'Track down Irena and kill her'.

I go into the room (full of boxes and pipes etc) and clear it out. The team hacks the computer and more bad guys ambush the room. About a minute later I get a mission failure saying I failed to kill Irena

WTF am I doing wrong?

This mission sort of rings a bell? Is it one of the missions before you actually go to Las Vegas?

this is really starting to get on my tits now. I kill everyone in the room and more come down from the room and it still results in mission failure.
