If someone's gone to the trouble of writing an automated spamming script, it's not going to be difficult for them to figure these things out automatically.

As it's proven to be robust in the past, you could have a number of challenge questions which alternate randomly. The problem is that these spammers are determined little fuckers and answering questions has been made quite easy for computers by Google and Wolfram Alpha's APIs. Even a challenge picture with a what's this type question could be answered by a computer because of image searching.
