Makes no sense really, but I get the gist. I didn't even think you could get turned down for a phone contract They gave me one

Your credit report won't tell you WHY you've been turned down for <whatever>, only the creditor will be able to tell you (which they obviously don't).
The only thing you will see on a credit report is the search by O2, and that will probably be on an Experian credit report IIRC, not Equifax.
Your best bet is to go for a 30 day SIM-only contract which they should give anyone with crap credit (unless you are a fraudster LOL) and then you can upgrade to a "proper" contract in say 3 month's time.
This is what I did BTW, I was originally on PAYG, went to 30 day SIM only, and now on a proper contract - and my credit history is quite bad (2006 was not a good year for me ).

How bad is "bad"? Missed payments? Default? CCJ?

Took me 4 years to sort out my credit, couldn't get shit all. Enough missed payments on Loans will cause bad credit, defaults and that sort of thing. Also the more times that you apply for credit the more it scores against you.
