Quite a bit.
Lock screen clock/calendar is a standard winterboard package that needs SHSH'ing to the phone with a custom HTML background. Then 'move to unlock' to hide the standard clock and unlock bar.
Springboard widget can be had off Macthemes, again you need to SHSH to the phone and edit a bit of code for the correct city location and position on the screen.
Icon placement you need Iconoclasm then again edit the code to get them lined up on the shelves. Winterboard is a must, hide docked and undocked icon labels, hide page dots, you need Spring clean to hide the clock, carrier name and battery and 'transparent status bar' to make it look a bit cleaner. Icons are compiled from various themes and SHSH''d to the phone.
Went from a completely standard setup to that last night, took about 3 hours in total but was bored so thought fuck it.
You can press the button at the bottom of the lock screen to unlock or just swipe the screen out the way:
Got the transition between pages set to curl and roll away too which is pretty cool watching all the icons line up on the shelves individually.

If you're thinking about doing it have a read up on Macthemes first, all the downloads are on there and most come with a readme file with instructions on how to use.

Changed up the store icons and put them on the dock, changed the notification badges for something more subtle and also changed the battery for a minimal one. Just want to do the Signal and Wifi and I'm finished
Quite hard finding out where all the files are and what to name them as a lots been changed round in iOS5 so when you download stuff all the filenames need to be changed. Keeps me busy while the mrs watches big brother though.

It's not an app, its called a 'Springboard widget' there are loads knocking about. You need to be jailbroken.

Not really no. Just do a backup before you start then you can always restore.
